How to avoid back pain from sitting all day, by moving through it

Guidance on back pain does not tell you to sit less or sit better. It tells you to stay active and keep doing normal things, and short movement breaks are the easiest way to do that at a desk.

Before you start

See a doctor rather than starting these if the pain followed a fall or accident, comes with fever, unexplained weight loss, or weakness in a leg. Stop and speak to a clinician if a movement brings on sharp pain, dizziness, or symptoms that travel down an arm or a leg, and seek urgent care for any loss of bladder or bowel control or numbness around the saddle area.

What the guidance asks for

NICE's low back pain guideline asks for self-management advice that includes encouragement to continue with normal activities, and for exercise chosen around what the person can and will do. The NHS back pain page says the same in plainer words: stay active rather than resting.

Neither says a chair is the problem. Both point at the same remedy for a long day in one: do something else for a few minutes, often.

Why small breaks, and not one big session

Cochrane's 2021 review of 249 trials found exercise more effective than no treatment, usual care or placebo for chronic low back pain, with only small differences between types of exercise. When the type matters little, the best choice is the one you will actually repeat, and a minute at the desk gets repeated more easily than a trip to the floor.

What this page cannot tell you

Nothing here can say why your back hurts at the end of a desk day. Sources agree that movement helps people with back pain, and they do not agree on one position or one stretch that prevents it. The sequence below moves the spine through different directions and wakes up the legs and hips, which is the part guidance supports.

The sequence

  1. 1

Seated cat cow · 45 to 60 seconds

Moves the spine through both directions without leaving the chair. Do it after every long stretch of typing.

  2. 2

Seated twist · 30 to 45 seconds each side

Rotation is the direction a desk never asks for. Turn from the middle of the back and stop early.

  3. 3

Seated overhead reach · 30 to 60 seconds

Both arms up and the ribs lifting, a quick way to undo the folded desk position.

  4. 4

Sit to stand · 30 to 60 seconds

Standing up from the chair on purpose, slowly, wakes up the legs and hips that have been switched off.

  5. 5

Supported low lunge · 30 to 60 seconds each side

Opens the front of the hips, which spend the day bent at the chair.

  6. 6

Thoracic extension over a chair back · 45 to 90 seconds

The upper back bending the opposite way to a desk, with the chair back as the support.

Pick two or three and repeat them through the day. Short and often beats one long session, and nothing here should send pain down a leg.

That sequence was written for everyone with this. Yours would be built around your answers.

What the research says

NICE NG59 recommends self-management for low back pain, including encouragement to continue with normal activities, and a group exercise programme (biomechanical, aerobic, mind-body or a mix) chosen around the person's needs, preferences and capabilities.

The NHS back pain page advises staying active rather than resting, and lists the symptoms that need emergency care rather than exercise: numbness or weakness in both legs, loss of feeling around the genitals or anus, and changes in bladder or bowel control.

Cochrane's 2021 review of 249 trials found moderate-certainty evidence that exercise is more effective than no treatment, usual care or placebo for pain in chronic low back pain, with only small differences between types of exercise.

Common questions

How do I avoid back pain from sitting all day?

Break the day up with movement. NICE advises people with back pain to continue normal activities and to choose exercise they can keep doing, and the NHS advises staying active rather than resting. A minute of seated cat cow, a twist or a sit to stand every so often fits both.

Does sitting all day cause lower back pain?

We cannot say that from here, and the guidance we cite does not claim it. What it supports is that exercise helps people who have back pain. If your pain follows a long day in a chair, moving more through the day is the part with evidence behind it.

What is the best stretch for back pain from sitting?

There is no single best one. Cochrane found only small differences between types of exercise for chronic low back pain. A seated cat cow, a seated twist and an upper back extension over the chair back cover three different directions in about two minutes.

How often should I get up when I sit all day?

The sources we use do not set an interval, and we will not invent one. A practical rule is to move whenever you notice you have stopped, and to make the movement something you will repeat tomorrow.

When is back pain from sitting something to get checked?

Get urgent care for numbness around the groin or back passage, changes in bladder or bowel control, or weakness in both legs. See a doctor first if the pain followed a fall, comes with fever or unexplained weight loss, or travels down a leg.
