Lower back
Why does my back hurt when I wake up, and what to do first
Waking up stiff or sore is common, and no page can tell you why yours is. What guidance supports is moving rather than resting, so the first minutes out of bed are a good place to start.
This isn’t a workout to push through. It watches how your body responds and adjusts as it goes.
Before you start
See a doctor rather than starting these for pain that wakes you at night whatever position you are in, that followed a fall or accident, or that comes with fever, unexplained weight loss or weakness in a leg. Stop and speak to a clinician if a movement brings on sharp pain, dizziness, or symptoms that travel down an arm or a leg, and seek urgent care for any loss of bladder or bowel control or numbness around the saddle area.

Start your first sessionWhat we can and cannot say
Back pain on waking has more than one possible explanation, and nothing on this page can tell which one is yours. That is why this page does not guess. It covers what to do in the first few minutes, and when the pattern is worth having assessed.
Move rather than rest
The NHS back pain page advises staying active rather than resting, and NICE's guideline encourages people to continue with normal activities. If your back is sorest first thing, a few small movements before you stand give it something to work with. Directional preference, where repeated movement in one direction eases the pain, is described in a 2024 review as present in roughly 60 percent of people with chronic low back pain when trained therapists assess it, which is why the sequence below goes in several directions and lets you notice which one feels better.
When stiffness is worth a closer look
NICE's osteoarthritis guideline lists morning stiffness lasting longer than 30 minutes as an atypical feature when diagnosing osteoarthritis, which is a reason to have long morning stiffness assessed rather than stretched through. Pain that wakes you at night regardless of position, or comes with fever or weakness, is the same kind of signal.
The sequence
- 1
One knee to chest · 30 to 60 seconds each side
The gentlest first move: one knee in, the other leg resting, still lying in bed.
- 2
Small supine twist · 1 to 2 minutes
Knees tip a short way to each side and stop early, a small rotation before you put weight on the spine.
- 3
Mini bridge · 20 to 60 seconds
A low lift of the hips that wakes the muscles behind them. Keep it very small on a soft mattress.
- 4
Small cat cow · 45 to 60 seconds
On hands and knees, a short movement through both directions, so you can notice which one feels easier.
- 5
Quick child's pose · 30 to 60 seconds
A short rest in a forward fold to finish. Skip it if folding forward is the movement that hurts.
Go slowly and keep every movement small. If one direction eases the pain and another makes it worse, do more of the first and skip the second.
That sequence was written for everyone with this. Yours would be built around your answers.
Start your first sessionWhat the research says
The NHS back pain page advises staying active rather than resting in bed, and lists the symptoms that need emergency care: numbness or weakness in both legs, loss of feeling around the genitals or anus, and changes in bladder or bowel control.
NICE NG59 recommends tailored self-management for low back pain, including encouragement to continue with normal activities, and exercise chosen around the person's needs, preferences and capabilities.
A 2024 systematic review with meta-analysis of the McKenzie Method for chronic low back pain found directional preference, sustained symptom reduction after repeated movement in one direction, in roughly 60 percent of people when assessed by trained therapists.
NICE NG226 (osteoarthritis in over 16s, 2022) lists morning joint stiffness lasting longer than 30 minutes as an atypical feature when diagnosing osteoarthritis in people 45 or over.

Osteoarthritis is diagnosed clinically in people 45 or over with activity-related joint pain and either no morning joint-related stiffness or morning stiffness lasting no longer than 30 minutes; prolonged morning stiffness is listed as an atypical feature.
Common questions
Why do I wake up with back pain?
There is more than one possible reason, and we cannot tell which applies to you from here. What the guidance we cite supports is staying active rather than resting. If it follows a pattern, such as long morning stiffness, that is worth raising with a clinician.
What should I do when I wake up with lower back pain?
Start with small movements while still lying down: one knee to the chest, a small twist, a mini bridge. The NHS advises staying active rather than resting in bed, and these are an easy way to begin before you stand.
Is morning back stiffness normal?
Some stiffness on waking is common. NICE lists morning stiffness lasting longer than 30 minutes as an atypical feature when diagnosing osteoarthritis, so stiffness that goes on longer than that is worth having assessed.
Should I stretch or rest when my back hurts in the morning?
Move gently rather than rest. NICE encourages continuing normal activities and the NHS advises staying active. Keep movements small, and stop any that send pain down a leg.
When should waking up with back pain be checked?
Get urgent care for numbness around the groin or back passage, changes in bladder or bowel control, or weakness in both legs. See a doctor first for pain that wakes you at night regardless of position, follows a fall, or comes with fever or unexplained weight loss.
