Neck pain, and the muscles nobody trains
Neck pain is rarely about the neck alone: the shoulder girdle and upper back set the position the neck has to hold all day.
What it is
Neck pain usually builds from position and load rather than from an injury. Hours of looking down or forward leave the muscles at the back of the neck long and overworked and the ones at the front short and quiet, while the deep stabilisers that should hold the head over the shoulders stop contributing. The result is stiffness, a reduced range when you turn, and often headaches that start at the base of the skull.
How it behaves day to day
It tends to be worse late in the day and after long screen sessions, and better after movement or a change of position. Many people notice one side turns further than the other. Tension headaches and a tight, gripping feeling across the tops of the shoulders commonly travel with it.

- Long stretches of looking down at a phone or laptop
- A screen set too low or off to one side
- Carrying a bag on one shoulder consistently
- Sleeping in a position that leaves the neck side-bent for hours
What the research recommends
The reference document is the American Physical Therapy Association's neck pain clinical practice guideline, revised in 2017 and published in JOSPT. It classifies neck pain into groups rather than treating it as one thing: neck pain with limited motion, neck pain with headaches, neck pain with movement coordination impairments including whiplash, and neck pain with radiating arm symptoms. Its treatment recommendations are consistent across those groups on one point in particular: clinicians should provide neck and shoulder girdle endurance exercise. Endurance is the word that matters. The deep muscles involved are not trained by stretching, and they are not trained by occasional strong effort either, which is why low-load holds repeated regularly are the shape the guideline describes.

Common questions
Is my posture causing this?
Position and sustained load matter, but posture is less about one correct shape and more about how long you hold any shape. The guideline emphasis is on endurance in the neck and shoulder girdle, which is what makes a position sustainable.
Will stretching fix it?
Stretching often gives short-term relief without changing what caused it. The APTA guideline recommends endurance exercise for the neck and shoulder girdle, which is the part most home routines leave out.
Can neck problems cause headaches?
